My maiden name is.. well my maiden name... xyz
My married name (official) is a hyphenate.. xyz-abc
My married name common is just my husband's last name... abc

I didn't even bother tossing my middle name in there >:) Nor did I bother with the Jackie/Jacqueline thing. Just stuck with Jacqueline
